Step 1
Please tick these boxes as follows if:
| Box 1: | Your child is entering Kindergarten, Year 3 or Year 7; |
| Box 2: | Your child will now be travelling to and from two homes because of a shared custody situation (and see FAQ); |
| Box 3: | Your child has moved address*; |
| Box 4: | Your child has changed school or campus*; |
| Box 5: | Your child has changed their name or wishes to be known by a new name*; |
| Box 6: | To correct an error on their current pass such as a misspelling or wrong information; |
| Box 7: | For any other reason, please tick the final box and then indicate under section four what this reason was. Please note that if your child has lost their pass, this is not the appropriate form to obtain a new one (see FAQ). |
*In the case of a change of address, school or name, please provide the previous details in Section 4.
Step 2:
What to enter in the following boxes:
| Box 1: | If your child is living in Australia on a visa, please supply the category number in the first box, otherwise leave blank; |
| Box 2: | Your child's last name/surname; |
| Box 3: | Your child's first and middle names (if any); |
| Box 4: | Your child's sex – M/F |
| Box 5: | Your child's date of birth. Please make sure you do not accidentally put your own date of birth or the current year i.e. 2012. |
| Box 6: | A good phone number to contact you on during the hours of travel. If you wish to supply additional phone numbers, you may put this in Section 4. |
| Box 7: | The mother's maiden name. This is purely a security measure and helps distinguish between children of the same name. |
| Box 8: | The child's primary address. Please ensure you provide a street number where possible and not just an RMB or property name. This address must not be that of a part-time carer or a relative. (See FAQ) |
| Box 10: | If your home address is not the same as your postal address, please provide this in box 10 as we do often need to send correspondence via the mail. |
Step 3:
| Box 1: | If this travel is for the current year (2012), please tick "THIS year"; |
| Box 2: | If the travel is for 2013, please tick for "NEXT year"; |
| Box 3: | Please circle which year your child is in. If the travel is for 2013, please circle the year that the child will be going into in 2013. |
| Box 4: | Please enter the name of the school that your child will be travelling to, and specify the campus if there is more than one. |
Step 4:
If your child has changed school (or campus), home address, their name, or if you are submitting a new form for any other reason, please fill this information in here.
Step 5:
This section is only relevant for students with irregular journeys.

If your child uses a rural bus service before connecting with a Deane's or Transborder bus, please indicate this here along with the transfer location. For a list of connecting services, please see the FAQ page.
Step 6:
This section must be filled out in order for this form to be valid. If your child is under the age of 16, the parent/ of guardian must confirm that you have explained the terms and conditions of travel to your child and you both abide to them. If your child is over the age of 16, they must do this themselves. Please ensure that the date box is filled in.
Step 7:
This section must also be filled out in order for the form to be valid. If it is not filled in, it will be sent back to the school and this will cause delays in processing your child's pass on time. Please make sure the school principle or relevant staff member have signed this section if you are personally sending this form to our office.
